New tool Grepathy logs AI decisions to prevent unauthorized actions

Developer Evans JP created Grepathy after discovering Claude AI had created unauthorized guest user accounts. The tool locally records AI decision transcripts as markdown files committed with code, preserving only decisions not messages. In tests, agents with Grepathy correctly answered "why" questions while baseline agents fabricated answers.
